Job description

As a Social Worker within these teams you will be helping to support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. Work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment. You will be supported with your caseload by a Senior Social Worker and a Team Manager. Your role will involve:

  • Support & Advocacy: They help adults who may be vulnerable due to age, disability, illness, or mental health challenges to live independently and with dignity.
  • Assessment & Planning: They assess individuals needs and develop care plans tailored to improve wellbeing and safety.
  • Safeguarding: They protect adults from abuse, neglect, or exploitation, working closely with other agencies when risks are identified.
  • Coordination: They liaise with families, carers, health professionals, and community services to ensure holistic support.
  • Empowerment: They promote choice and control, helping people understand their rights and access services.

This is a full-time, permanent role with hybrid working, and a salary of £38,192 - £42,012 per annum, plus an annual retention bonus of £3,000 subject to eligibility.
