The North Locality Team in Denbighshire is offering an exciting opportunity for an Agency Social Worker within the Adult Social Care and Homelessness Service. Get ready to immerse yourself in a dynamic environment at the Rhyl Community Resource Team where health, social care, and third-sector professionals come together to deliver outstanding integrated services. This temporary role of six weeks offers 37 hours a week on a full-time basis. As part of your role, you will carry out assessments to ensure that individuals and their families can achieve their desired outcomes. A professional qualification in Social Work and registration with the Care Council for Wales are essential for this position.

What you will do:

• Collaborate with health, social care, and third-sector teams to deliver integrated services to the local community.
• Conduct appropriate and proportionate assessments for individuals and families to help them achieve realistic outcomes.
• Prioritise and manage your workload efficiently while working alongside Occupational Therapists and other social care staff.
• Engage in proactive planning to ensure effective partnership working within the community.
