Oxfordshire Council is currently recruiting for a Permanent Adult Social Worker.

Job Purpose:

This post is based in Adult Social Care. The post holder will work collaboratively across adult social care and partner agencies to support and promote strong communities, making a real difference to the people and communities we serve, now and in the future. The role involves delivering value for money and supporting individuals through a strength-based approach to create opportunities, build resilience, and establish long-term support networks, enabling people to live as independently and safely as possible.

The post holder will work within the competency framework, achieving progression to Grade 11 within the specified timescales.

Job Responsibilities:

• Work within the requirements of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and other relevant legislation, providing personalized information and advice to individuals, families, and carers.
• Use strength-based approaches, exploring all options including informal support networks, community resources, and assessments for equipment and adaptations to meet identified outcomes.
• Undertake timely and proportionate assessments, reviews, and professional interventions related to individuals, families, and carers.
• Ensure support plans reflect the persons' needs and outcomes with a clear breakdown of how these are being met, regardless of eligibility determination.
• Identify and arrange services that improve and maintain independence, such as reablement services and equipment, including:

- Assess and provide equipment, including Assistive Technology, to meet needs and reduce risks.

- Assess and advise on minor and major property adaptations.

- Assess and advise on moving and handling techniques, including postural management support, and issue appropriate equipment.

- Collaborate with housing partners to assist with re-housing applications in accordance with local policies.
