Role Responsibilities:

Deliver holistic assessments, care planning, and reviews under the Care Programme Approach.

Work collaboratively with service users, families, and professionals to establish robust support networks.

Advocate for individuals with complex mental health needs, promoting autonomy and recovery.

Apply legislation such as the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and Human Rights Act 1998.

Fulfil statutory social work functions, including safeguarding responsibilities.

Promote early intervention to reduce the risk of crisis through effective signposting and support.

Participate in multi-disciplinary meetings, contributing social work insights and expertise.

Encourage a strengths-based, solutions-focused approach to drive meaningful change.

Candidate Profile:
