Job summary

Charles Hunter Associates Ltd is seeking a qualified Social Worker to join our Hospital Discharge Team, focusing on safe discharge of adult patients with ward and home assessments.

You will assess needs to ensure independence and plan services in a fast-paced environment. A Degree/DipSW/CQSW and at least 2 years post-qualified experience are essential.

£32.00 per hour umbrella (PAYE options available), with parking nearby and easy access by car or public transport.

About the team

The team are responsible for the safe discharge of adult patients from Hospital,

You will be responsible for the assessment of each patient ensuring that all their needs are met and that they can continue to live their lives as freely as possible. this position involves assessments from home as well as being present on the wards when necessary.

About you

The ideal Social Worker will be able to manage time effectively to prioritise their workload as well as have good interpersonal skills. Experience working with adults, the ability to work in a fast-paced environment and the ability to be able to engage with service users from diverse backgrounds is key to the success of the role. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years years experience is essential in order to be considered for this role.
