Job summary

Charles Hunter Associates Ltd is seeking a Social Worker to join our Hospital Discharge Team in Portsmouth. The role focuses on safe discharge planning, assessing each patient’s needs and ensuring they can continue to live as independently as possible, with assessments conducted both at home and on the ward.

The ideal candidate will have a Social Work qualification (Degree/DipSW/CQSW) and at least 2 years post-qualified experience, be able to manage time effectively in a fast-paced environment,

Qualifications

• Ability to work in a fast-paced environment and with adults from diverse backgrounds.
• Degree within Social Work (Degree/DipSW/CQSW) with at least 2 years' experience is essential.

Responsibilities

About the team

The team are responsible for the safe discharge of adult patients from Hospital,

You will be responsible for the assessment of each patient ensuring that all their needs are met and that they can continue to live their lives as freely as possible. this position involves assessments from home as well as being present on the wards when necessary.

About you

The ideal Social Worker will be able to manage time effectively to prioritise their workload as well as have good interpersonal skills. Experience working with adults, the ability to work in a fast-paced environment and the ability to be able to engage with service users from diverse backgrounds is key to the success of the role. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years' experience is essential in order to be considered for this role.
