In this dynamic role, you will be earning £32 per hour and have the work flexibility of Full Time/Part Time options. Our team supports Southwark’s @Home/Urgent pathway, a crucial hospital admission avoidance service. What you will do:

– Intervene with residents who can be supported home from hospital emergency departments to avoid admissions.
– Manage a caseload of 5-7 cases, ensuring each is progressed efficiently within a 10-day timeframe.
– Collaborate with the multi-disciplinary team to stabilise residents, implementing urgent care plans and transferring cases to Reablement.
– Complete Care Act Assessments, develop Care and Support Plans, and review existing care arrangements.
– Engage in safeguarding, mental capacity assessments, and Best Interests Decision Making.

Requirements:

– SW qualified and registered, with at least 2 years post-qualifying experience(post ASYE).
– Experience in fast-paced services like Urgent Response or hospital discharge is desirable.
– Understanding of Reablement ethos and strengths-based practice is an advantage.
