This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South-West London & St George’s Mental Health Trust (SWLSTG).
The FIND Team works closely with the Oak Unit, a Learning Disability and Autism (LDA) Ward, which is part of the South-West London & St George’s Forensic Service. The team provides specialist assessment, treatment, and consultation to ensure a safe and clinically justified pathway for individuals with mental illness, offending history, and learning disabilities, with or without neurodevelopmental disorders.
The three forensic services of SWLSTG, South London and Maudsley (SLaM), and Oxleas form the South London Mental Health and Community Partnership (SLP). The FIND Team collaborates with other Trusts to facilitate service-user transitions from inpatient settings to community care.
We are seeking a senior social worker to provide social work input within the FIND Team for SWLSTG. The role involves supporting service users in the community, managing transitions from hospital, acting as a point of contact for CPA, monitoring discharges, and providing care coordination.
This is a full-time position offering flexible and agile working arrangements. The post-holder will ensure adherence to relevant policies and procedures and contribute to the smooth operation of the service.
