Job summary

Jobtailor is recruiting a Qualified Social Worker in East Sussex to cover Lewes and surrounding areas. You will work with Adult Social Care’s Out of County Hospital Team, supporting safe discharge from hospital to people’s homes and promoting the home first model.

Travel across East Sussex, including rural areas, and collaborate with hospital trusts, reablement teams and Integrated Commissioning Boards to help people access continuing care funding.

• Evidence of an overseas police check if lived or worked abroad

• Occasionally undertake work for the Community Hospital Social Work Team covering Eastbourne and Hastings

Requirements

• Qualified Social Worker


• Registered Social Worker with Social Work England


• Social work qualification


• Completed Assessed and Supported Year in Employment or equivalent if prior to 2012 (NQSW year and academic module)


• Ability to meet travel requirements across East Sussex, including rural areas


• Resilience and flexibility to manage a dynamic and complex caseload


• Knowledge of appropriate legal and policy frameworks informing and mandating social work practice


• Enhanced DBS check or Enhanced check for Regulated Activity required


• Evidence of an overseas police check if lived or worked abroad


• Applicants requiring UK work sponsorship must disclose this so eligibility can be checked
