Job summary

Lambeth Council is seeking a qualified social worker to join our Information Contact and Assessment Service (ICAS). The role combines on-site work with a flexible hybrid arrangement (3 days per week on site, 2 days at home) to support adults with complex needs.

You will deliver strength-based, person-centred preventative care, conduct Care Act assessments, and contribute to robust care planning while collaborating with several teams and community partners.

Qualifications

  • Qualified social worker with current registration.
  • Experience with s.9 Care Act Assessments essential.
  • Ability to work in a hybrid model: on site 3 days a week and 2 days at home.

  • Adapt to a demanding environment and commit to partnership and collaborative working.
  • Deliver strength-based, person-centred preventative support and complete robust care planning.
  • Participate in the duty rota, screen routine and priority referrals, and complete urgent home visits.
  • Knowledge of Occupational Therapy equipment and minor adaptations; be a Trusted Assessor or undertake training.
  • Signpost to other services and provide information and advice.
  • Identify safeguarding issues and respond timely, following safeguarding procedures.
  • Manage cases at pace to achieve positive outcomes for residents.

About the Team

Information Contact and Assessment Service; ICAS, is a front door social care team where workers complete assessments, initial safeguarding concerns, mental capacity assessments, preventative assessments, and assess and review low level equipment provision. We triage a large volume of referrals from a variety of sources which puts huge demands on our duty service. We need to be knowledgeable about the range of teams within adult social care, their remit and referral routes. We need to be aware of current community resources to support our preventative offer and to work co-operatively and collaboratively with our colleagues in AgeUK Lambeth.

About the Role:

We are looking for a qualified social worker to join our front door team- Information Contact and Assessment Service [ICAS].

  • You will work closely with other teams in the borough and engage wider agencies to support the person.
  • You’ll need to adapt to a demanding and changing environment and have a commitment to partnership and collaborative working.
  • The team support hybrid working, the requirement is to be on site 3 days a week with the option to work 2 days at home.
  • The team carry out s.9 Care Act Assessments with adults over 18 years of age, with a range of physical disabilities and health vulnerabilities, so experience with these client groups is essential.
  • The primary feature of work undertaken by staff is to deliver strength based, person centred preventative support and to complete robust care planning for eligible people.
  • The Social Worker will participate in our duty rota where we screen both routine and priority referrals and complete urgent home visits
  • The Social Worker will need some knowledge of Occupational Therapy equipment and minor adaptations, be a Trusted Assessor, or happy to undertake this training
  • The Social Worker will make effective use of resources through care planning, signposting to other services, or providing individuals with information and advice.
  • The Social Worker will identify and provide a timely response to safeguarding issues, evidencing your robust understanding of safeguarding procedures.
  • The Social Worker will have the ability to work at a pace, and safely manage cases to achieve positive outcomes for our residents
