Job Title: Learning Disabilities Adult Social Worker
Location: Hackney
Hours: Full-time, 36 hours per week
Pay Rate: £32 per hour

What you will do:

• Serve as the Duty Social Worker for the Integrated Learning Disability Team, providing seamless and excellent health and social care services.
• Independently manage urgent matters by utilising a range of resources to ensure the safety and wellbeing of service users with learning disabilities.
• Conduct assessments and reviews, responding swiftly to emergencies and being the first point of contact for safeguarding concerns.
• Ensure clear and robust documentation of actions and decisions on the Mosaic system.
• Uphold risk management protocols and apply the principles of the Mental Capacity Act where appropriate.
• Deliver top-tier social care practice, ensuring quality interventions, solid decision-making, and adherence to the Standards of Social Work England.
