Job summary

Nottinghamshire County Council in Mansfield is seeking dynamic and compassionate Social Workers for the Urgent & Emergency Care Team across Mansfield, Ashfield and Newark.

You will support adults through hospital discharge and recovery with a person-centred approach, working in a hybrid pattern to balance caseloads and community need. The role requires registration with Social Work England and safeguarding knowledge.

Qualifications

  • A recognised Social Work qualification and registration with Social Work England.
  • Knowledge of adult social care legislation and policies.
  • Experience in adult safeguarding and working with health and external agencies.
  • Strong organisational skills to manage a mixed caseload.

Responsibilities

  • Collaborate within an integrated multidisciplinary team at the Transfer of Care Hub.
  • Make informed discharge pathway decisions for individuals needing support.
  • Conduct Care Act assessments, Mental Capacity Act assessments, and Safeguarding enquiries.
  • Apply critical thinking and professional judgement to manage risks effectively.
  • Foster independence, wellbeing, choice, and control for adults requiring support.
  • Build positive relationships with individuals, families, and professionals to achieve person-centred outcomes.
  • Manage a varied caseload in a fast-paced environment while upholding dignity and human rights.

We are seeking dynamic and compassionate Social Workers to join our Urgent & Emergency Care Team in Mid Nottinghamshire. As a Social Worker, You Will

To Succeed In The Role You Will Need
