Job summary

Derby City Council is recruiting a Community Care Worker to join the Citywide Response Team. The role focuses on triaging referrals, conducting community assessments, and creating personalised support plans to improve residents' health and wellbeing.

Working in a team with flexible hours (29.4 per week) and a permanent contract, you will engage with people, carers and professionals to determine needs, urgency and appropriate actions within social care.

Qualifications

  • Excellent communication and listening skills.
  • Understanding of the Care Act 2014 and able to implement a strengths-based approach.

Derby City Council is an innovative and forward-thinking Council, committed to developing practical and proportionate ways of undertaking social care assessments, developing support plans and interventions to enable the residents of Derby to live fulfilled lives, achieving their outcomes.

An exciting opportunity has arisen enabling the recruitment of a Community Care Worker to join the Citywide Response Team. The teams' aims are to support a stable and consistent response to referrals received for adult social care support; and to support the changing priorities and demands across adult social care.

The successful candidates will work as part of a team on duty to triage and respond to referrals made to adult social care requesting support for people with social care needs. This will involve liaising with people, carers and professionals to ascertain details about presenting social care needs and the level of urgency, to develop interim plans to meet such needs and risks identified.

Alongside this work, the successful candidates will carry out social care assessments in the community; to support people and carers to create individualised support plans to meet their identified needs and outcomes. This will include working with people with a variety of primary needs in relation to Physical Disability, Autism, Older adults, homelessness and needs associated to addiction and people seeking asylum.

You will need to have excellent communication and listening skills, with the confidence and ability to negotiate and challenge appropriately. You will need to have an understanding of the Care Act 2014 and be able to implement a strengths-based approach to your work with people, their carers and support networks. You will be able to manage a case load independently, and work as part of a supportive team.
