We are seeking a qualified and experienced Social Worker to join our Older Peoples Team. You will work with adults who have complex social care needs, managing a varied and increasingly complex caseload with greater autonomy. Your role will involve safeguarding, assessments, care planning, and service delivery in collaboration with health and care partners. • Coordinate and review person-centred care plans.
  • Lead safeguarding enquiries.
  • Liaise with service users, carers, families, and multi-agency professionals.
  • Provide crisis intervention and emergency response when necessary.
  • Maintain accurate records and contribute to service evaluation and planning.

This post includes a market rate supplement of £2,000 per annum (pro rata), subject to review, a welcome payment of £6,000, and retention payments of £2,000 paid after 4 years of service and subsequently every 4 years.

Note: We are unable to consider applications from candidates based overseas.
