We are seeking a qualified and experienced Social Worker to join our Older Peoples Team. You will work with adults who have complex social care needs, managing a varied and increasingly complex caseload with greater autonomy. Your role will involve safeguarding, assessments, care planning, and service delivery in collaboration with health and care partners. You will play a key role in promoting personalisation, ensuring services are tailored to the individual, supporting independence, and enabling older people to live fulfilling lives.
Key Responsibilities:
This post includes a market rate supplement of £2,000 per annum (pro rata), subject to review, a welcome payment of £6,000, and retention payments of £2,000 paid after 4 years of service and subsequently every 4 years.
