Nightingale Hammerson is seeking a passionate Social Worker for their palliative care team in Bristol. This role involves managing your own caseload, providing emotional support, and working with patients in various settings. The Social Work Team collaborates with patients, families, and carers who need palliative social work support. The role involves working with patients in inpatient units and community settings, including their homes.
The team works closely with families, carers, community nursing teams, hospice services, and external organisations to support people in the advanced stages of illness, helping them access necessary support and plan for the future.
The successful candidate will join a friendly and professional team that makes a real difference to patients and their families during challenging times. This role offers time to build trusting relationships and support in creative ways.
You will manage your own caseload, so the ability to work independently is essential.
We seek someone who can support patients, families, and carers with sensitivity and compassion.
You should have experience working with individuals approaching the end of life in health or social care settings.
A clean driving licence and access to a car are required, as the role involves travel across Bristol, South Gloucestershire, and North Somerset.

Due to the nature of the work, this position is exempt from the ROA, and the jobholder will be required to undergo a Disclosure and Barring Service (DBS) check.
A vaccination history or evidence of immunity is also required.

Salary / Benefits: Band 4 equivalent, £27,061 - £29,696 FTE, dependent on experience.
