Job description

The Community Mental Health Team (CMHT) for Older People is a multi-disciplinary team for adults over the age of 65 years who require specialist secondary mental health services due to experiencing symptoms of a severe and enduring mental health condition. The team consists of Mental Health Nurses, Social Workers, Support Workers, Occupational Therapists, physiotherapists, psychologists, and psychiatrists.

The team provides well-being assessments, care and treatment planning, occupational and physiotherapy, psychological therapy, and a range of psychological supports such as sleep hygiene, CBT, behavioural activation, medication monitoring, setting SMART goals, and safety planning according to the needs of the individual using the services.

The team tailors interventions to foster independence, well-being, and quality of life, ensuring comprehensive and personalised support during mental health recovery.

You will join a team that specialises in supporting older adults who require secondary mental health services. The team is committed to strengths based, preventative social work, supporting people to live fulfilled lives and to remain as independent as possible.
As a CMHT Social Work Assistant, you will have access to a comprehensive range of resources and support to continue your professional development and to help you become the best worker you can be.

The CMHT is part of Mental Health Services for Older People which is based on LLlandough Hospital, Llanfair Unit in Penarth.

What We Are Looking For From You

We are looking for motivated workers who are looking to gain further experience within Cardiff Council. As the successful candidate you will be able to work independently and manage your time. You will be able to show you have excellent engagement and information gathering skills in and the ability to write concise records and reports. You will be a good team player, have excellent communication, organisation skills and IT skills.

You will be supervised and supported by an experienced social worker who will offer mentorship and guidance on your cases and will support your career development within the team.

You will be committed to supporting individuals experiencing mental health challenges, working in a strength-based, preventative manner to provide the necessary support for them to achieve their goals.

You will be committed to working in a way that promotes equality and inclusion for all our citizens.

The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.

This post is part time and will work 32.5 hours per week.
