A local independent charity is seeking a Social Prescribing Team Service Manager to oversee NHS-funded services in London. You will lead a team of 22 staff, manage stakeholder relationships, and develop service quality. The ideal candidate should have experience in service development and managing teams, alongside strong communication skills. Age UK Barnet is looking for anexperienced manager to lead our Social Prescribing service which includes anAgeing Well MDT project.
As our Social Prescribing Team Service Manager, you will oversee the delivery of keyNHS-funded services that help local residents stay well and independent. Youwill lead and support up to twenty-two staff across two contracts, build strongrelationships with stakeholders, develop and grow the services, and ensure allKPIs are met.
The Social Prescribing Service Manager role spansthe management and leadership of NHS projects including the Aging Well MDT anda team of twenty-two Social Prescribing Link Workers (SPLW). The aim is tosupport the community to stay well and increase their independence byaddressing non-medical determinants of ill health. Positioned within AgeUK Barnets core advice team and reporting directly to the Advice ServicesManager, the role forms a key part of the portfolio of contractual services andgrant-funded projects delivered by Age UK Barnet.
This is a full time roleleading and supporting the social prescribing link worker team across the twocontracts, managing key stakeholder relationships, developing services andpromoting awareness across the borough as well as delivering all KPI requirements.
Depending on the skills and experiences of theappointed candidate, this post holder may take on additional duties andresponsibilities within Age UK Barnet.

Our Social Prescribing team work at GP Practices throughout the Borough of Barnet and have increased to a team of 22 link workers over the past few years.
Service Development:
Tocontribute to the strategic planning and operational running of NHS linkedprojects including the SPLW service.
Tocontribute to the development and maintenance of systems to improve servicedelivery and ensure quality of care and support.
To promoteintegration and the benefits of collaborative voluntary sector services andprojects to the wider staff team, and to VCS colleagues in the borough.
Todevelop excellent working relationships across the relevant NHS structures andto navigate within a complex partnership arrangement
In respect of other services:-
Tocontribute to the development of Age UK Barnet services inprimary care and other service initiatives aimed at enabling older people tostay well and to retain their independence.
Staff Management:
To lead the recruitment of staff and ensure that skilled andexperienced staff resources are in place to meet demand for the services.
To oversee and support the deployment of SPLWs into PrimaryCare Networks.
To identify and oversee the planning and implementation ofappropriate staff training including induction and in-service training.
To support the develop management arrangements of theworkforce, escalating care and support delivery issues and risks as appropriate.
To manage staff across the two services, ensuring thatsupervision and appraisals are in line with organisational policy adhering toagreed service standards and objectives.
To lead on the management of staff disciplinary issues inline with organisational policy.
To ensure that safe working practices are in place for allstaff.
To manage team and staff relationships for positive outcomesand to liaise and coordinate with GPs and key commissioners on staffing issuesassociated with the employment of SPLWs.
To lead peer to peer support sessions anddeliver 1-1 sessions with SP staff every month alongside the deputy manager.
To provide guidance and best practice advice tothe SP team.
Service Delivery and Quality Assurance:
To complete reporting duties for the twocontracts requirements, monthly, quarterly and annually using both Access Elementaland Charity Log case management systems.
To support the implementation and operationaldelivery of care and support across the team to ensure processes are developedand implemented.
To manage the Service Level Agreements and staffcontracts with the PCNs and CLCH.
To manage the Access Elemental Licence Contract.
To contribute to the development of key performanceindicators for MDT, SPLW and AUKB services.
Implementing strategic vision and streamlining across teamsand SPLW PCNs.
Promoting SPLW and other services with GPs, partners and thewider Barnet community.
To participate fully in audits and inspections byaccredited/regulatory bodies and take the lead on implementation of qualitysystems where appropriate.
Deliver information to help PCNs, commissionersand local partners identify unmet needs within the community and gaps incommunity provision.
Maintain budgets for existing services and any new servicedevelopments alongside the Finance Team and CEO.
Actively monitor budgets to deliver services within theagreed financial framework and resources.
Networking and Collaboration
To manage and maintain strong key stake holderrelationships for both services including hosting the SP steering group meetingmonthly and maintaining excellent working relationships with PCN leads and CLCHteam leads.
To network and collaborate with the VoluntarySector across Barnet.
To create material for and attend, networkingcommunity or PCN events when requested to promote and raise awareness of theservice.
To oversee and manage the project work assignedfor SP service development.
This role has various seats on regularPartnership meetings such as Making Every Contact Count, Healthy Hearts, BarnetHomes MDT, Barnets Community Connectors, London SP managers, SocialPrescribing Advisory Group (NCL), NCL Personalised Care, Barnet Faculty Leads(PCN), and Faculty of Educators (PCN).
Represent the organisation at external meetingsand partnership boards as required.
To be proactive in creating new relationshipswith new services across the borough, networking and seeking new ways toimprove collaboration with services that the teamwork with or have concernswith to improve patient care.
General Tasks
Datacapture
Work sensitively with people, families and carers to capture key information,enabling tracking of the impact of social prescribing on their health andwellbeing.
Encourage people, families and carers to provide feedback and to share theirstories about the impact of social prescribing on their lives.
Support referral agencies to provide appropriate information about the personthey are referring. Provide appropriate feedback to referral agencies about thepeople they referred.
Work closely within the PCN and Public Health Teams to improve the data setsoffered through Social Prescribing, ensuring all targets are achieved, and theimpact of the data is shared to those it will benefit as well as to those it isnecessary.
The post holder must always carry out duties and responsibilities with dueregard to the organisations equal opportunity policies and procedures.
Work as part of the PCN healthcare team to seek feedback, continually improvethe service and contribute to service improvement and business development.
Undertake any tasks consistent with the level of the post and the scope of therole, ensuring that work is delivered in a timely and effective manner.
Duties may vary from time to time, without changing the general character ofthe post or the level of responsibility.
The post holder may be required to complete evening and weekend work.
The post holder must always respect patient confidentiality and, in particular,the confidentiality of electronically stored personal data in line with therequirements of the General Data Protection Regulation (GDPR).
The post holder will be expected to take responsibility for self-development ona continuous basis, undertaking on-the-job training as required.
The post holder must be aware of individual responsibilities under the Healthand Safety at Work Act and identify and report as necessary any untowardaccident incident or potentially hazardous environment.
The post holder will ensure they accurately represent the PCNs and ensure thevalues of the PCN are always upheld in carrying out their work
The post holder will work as part of a team and provide cover for absentcolleagues.
The post holder may be required to undertake duties at any location within theBorough of Barnet, in order to meet service needs.
The post holder must always work in general accordance with the organisationspolicies and guidelines.
The post holder must always adhere to the organisations information governancepolicy, in particular ensuring that there is no breach of confidentiality as aresult of his/her actions.
