Job summary

Walsall Housing Group Limited is seeking a Social Prescribing Manager to lead the delivery of Social Prescribing and Young Persons Accommodation Support programmes, improving health, wellbeing and tenancy outcomes for vulnerable customers.

You will manage a team on-site, foster partnerships with councils and health services, monitor performance and ensure funding and contractual obligations are met, while driving equity and safeguarding across services.

We’re looking for a Social Prescribing Manager to lead the successful delivery of our Social Prescribing and Young Persons Accommodation Support programmes, helping us improve outcomes for some of the most vulnerable people within our communities.

This is a varied and rewarding leadership role where you’ll oversee services designed to improve health and wellbeing, prevent homelessness and help customers successfully sustain their tenancies.

As this is a front-line leadership role, you’ll need to work on site for a minimum of three days per week, working alongside your team and the customers and partners we support.

  • Lead and oversee the successful delivery of whg’s Social Prescribing programmes, including Ace in Asthma, and the Young Persons Supported Accommodation Scheme (YPSAS), ensuring contractual requirements, budgets and KPIs are achieved.
  • Provide compassionate and effective leadership to our Social Prescribers and Young Persons Temporary Accommodation Officers, supporting colleagues to deliver high-quality services and positive outcomes for vulnerable customers.
  • Lead the delivery of a non-medical social prescribing approach, connecting customers with community groups, activities and support services that improve health, wellbeing and independence.
  • Work in partnership with Walsall Council and other key partners to successfully manage YPSAS, supporting vulnerable young people to sustain their accommodation and progress towards general needs tenancies.
  • Build strong collaborative relationships with health and social care services, GPs, DWP, Local Authorities and community and voluntary organisations to improve customers’ access to services and support.
  • Work closely with teams across whg to ensure vulnerable customers have equitable access to the services and support they need to sustain their tenancies.
  • Oversee referrals and assessments for YPSAS, ensuring young people with appropriate support needs are identified and provided with the right housing management support.
  • Use data, wellbeing outcome frameworks and evidence-based approaches to measure the impact of both programmes, identify areas for improvement and ensure services reach customers experiencing the greatest inequalities.
  • Monitor programme performance and produce accurate monthly and quarterly management information and reports, highlighting achievements, outcomes and areas requiring improvement.
  • Manage programme contracts, grant claims and budgets, ensuring funding requirements and whg’s financial and procurement procedures are met.
  • Represent whg at multi-agency, partnership, contract and audit meetings, advocating for customers and contributing to the successful delivery and development of services.
  • Ensure appropriate safeguarding, health and safety and risk management arrangements are in place, responding effectively to concerns where customers may be at risk of harm, abuse or neglect.
  • Contribute as a member of the Stronger Communities management team, sharing knowledge and ideas and supporting the delivery of whg’s wider Social Justice Strategy and corporate objectives.
We're looking for someone who has:
  • Experience of working alongside and supporting vulnerable customers to live independently and successfully sustain their tenancies .
  • Experience of leading social housing, tenancy support or accommodation programmes supporting vulnerable groups, including care leavers, people experiencing homelessness, people with disabilities or long-term health conditions, and others facing disadvantage.
  • Experience of delivering programmes that improve the long-term outcomes and life chances of social housing customers and other vulnerable groups.
  • Experience of working with care-experienced or vulnerable young people, including those who may be at risk of homelessness.
  • A strong understanding of health inequalities and the wider social determinants of health, and how these can impact customers and communities.
  • Good operational knowledge and experience of social prescribing, health assessments and wellbeing outcome frameworks, tools and techniques.
  • Experience of leading, motivating and developing a team to deliver high-quality services and achieve individual targets and KPIs.
  • Experience of developing strong partnerships with community, voluntary and statutory organisations to improve equitable access to and take-up of services.
  • Experience of contract management and externally grant-funded programmes, including meeting funding conditions, monitoring performance and reporting against funder requirements.
  • Experience of managing budgets in line with financial regulations and organisational procedures.
  • Experience of safeguarding, health and safety and risk management, including completing and overseeing appropriate risk assessments.
  • Experience of using data and evidence to evaluate programmes, demonstrate impact and improve customer outcomes.
  • A relevant housing qualification, or a commitment to work towards one, or significant relevant experience within social housing and tenancy sustainment.
  • A demonstrable commitment to social justice and a passion for improving outcomes for social housing customers.
