Job summary

Together Co in Brighton is seeking a Social Prescribing Link Worker to manage a caseload and deliver a person-centred social prescribing service in GP surgeries and patients' homes or other venues.

You will work collaboratively with staff, volunteers and external partners, be highly organised, safeguarding-aware and proficient with MS Office and databases to keep accurate records. This role is 3 days per week.

Qualifications

• Ability to manage own caseload with accuracy and empathy.
• Excellent communication skills with diverse clients and partners.
• Understanding of safeguarding procedures and data protection.

Responsibilities

• Manage a caseload of clients and provide a person-centred social prescribing service.
• Deliver services within GP surgeries and/or in patients' homes or other venues.
