Job summary

Modality Partnership in Walsall is seeking a Social Prescriber to deliver holistic, person-centred social support within a busy PCN. You will listen to patients, empower them to access community groups and services, and manage a caseload with clinical supervision.

The role supports collaboration with Divisional Boards and the Modality Business Support Team to drive innovative primary care practices. The position is a 1 year fixed-term contract, full-time, based in Walsall with flexible working

Qualifications

• Excellent communication skills and ability to listen and support patients.
• Ability to empower patients to access social and community services.
• Experience in social prescribing or primary care is desirable.

Responsibilities

• Provide social support to patients across the PCN.
• Carry and manage a caseload of patients, working independently.
• Collaborate with Divisional Boards and Modality teams to improve services.
• Utilise link workers for holisitc patient support and referrals.
