Job summary

Modality Partnership is seeking a Social Prescriber for the Walsall Division on a 1-year fixed-term, full-time basis. You will provide holistic, person-centred social support to patients across the PCN, collaborating with health and wellbeing coaches and care coordinators.

You will manage a caseload, work with community groups and services, and benefit from clinical supervision while contributing to service improvements across divisions.

Qualifications

• NVQ Level 3 or equivalent
• Commitment to professional and personal development
• Training in motivational coaching and interviewing (advantageous)
• Knowledge of personalised care approach
• Understanding wider determinants of health and equality, diversity, inclusion
• Knowledge of IT systems and basic reporting

Responsibilities

• Provide social support to patients across the PCN
• Manage a caseload and work independently within remit
• Collaborate with divisional boards and business support teams
• Refer and connect patients to community groups and services
