We are recruiting for the permanent role of Social Care Practitioner in the D2A North & West and Inner-City East.
We are looking for individuals who recognize the value of multidisciplinary working and are keen to progress the model of service delivery.
The D2A Service works together to promote a whole service approach, collaborating with internal & external colleagues.
The work is fast-paced, including Safeguarding Adults enquiries, Care Act assessments, and a good knowledge of local services and voluntary agencies, including Tier 1 & 2.
Your role involves working under the statutory requirements of the Care Act 2014 to undertake support conversations, strength-based Care Act assessments, Carers Assessments, and reviews with citizens in need of support, helping them access appropriate support in line with department policies and procedures under supervision from a Manager, Senior Practitioner, or Social Worker.
You will undertake support planning with service users who have eligible care and support needs to access the support they require and meet outcomes that promote choice, control, and independence.
As part of your role, you will conduct Mental Capacity Assessments and Best Interests assessments within the statutory framework of the Mental Capacity Act 2005, with support from qualified Social Workers where needed.
The team comprises a Manager, 2 Senior Practitioners, 6 FTE Social Workers, and 6 FTE Social Care Practitioners. You will receive regular formal reflective supervision from your manager or senior practitioner.
You will ensure citizens receive the right professional expertise promptly, aiming to prevent or delay the need for care and support by maximizing independence, autonomy, community engagement, managing risks, and promoting health.
You will be provided with a full induction, training, and opportunities for professional development. The teams are primarily based in Bristol, with an agile working policy allowing for home working.
