Job summary

Wokingham Borough Council is seeking a Social Care Practitioner in Wokingham to provide person-centred support for adults, helping them live safely, healthily and independently in their communities. The role is permanent, 30 hours per week, with a clear development pathway and supervision from a social worker/occupational therapist.

Qualifications

  • Experience in health or social care, or in a people-facing role.
  • Good IT skills and ability to learn social care recording systems.
  • Ability to engage with people with different communication needs.
  • Commitment to inclusivity and diversity.
  • Understanding how health or disability affects wellbeing.

Responsibilities

  • Work with people to find out what is important to them.
  • Carry out strengths-based assessments and reviews of peoples day to day lives and their support arrangements.
  • Gather information from individuals, carers, professionals and advocates.
  • Write reports and keep accurate records.
  • Build positive relationships with residents and support diverse communication needs.
  • Work closely with colleagues as part of a team.
