Job summary

Wiltshire Council is recruiting a Social Care Practitioner to help people live independently and improve wellbeing across Wiltshire. You will manage your own caseload and work with customers, carers and partner agencies to deliver person-centred care and support positive life changes.

Collaboration with Social Workers, Occupational Therapists and multi-professionals will be essential to promote independence, choice and wellbeing using community resources, with county-wide travel as part of the

Qualifications

  • Must hold NVQ Level 4 in Health and Social Care or equivalent.
  • Experience in Health or Social Care settings.
  • Good communication and organisational skills.

Are you passionate about helping people live independently and achieve their goals? As a Social Care Practitioner, you will be at the heart of improving lives and wellbeing across Wiltshire. You will join a forward-thinking team that values creativity, collaboration, and professional growth - where your ideas truly matter.

In this rewarding role, you will manage your own caseload and work closely with customers, carers, and partner agencies to deliver person-centred care. Working collaboratively with, Social Workers, Occupational Therapists and multi-professional teams, you will promote independence, choice, and wellbeing using universal services and community resources.

As this is a fast-paced team, you will lean on your organisational ability to manage caseloads effectively and have excellent communication skills to be able to work collaboratively. In addition to a sound understanding of the Care Act 2014, you will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.

You will be providing outcome-focused assessments to identify the right support for positive life changes and creating and reviewing support plans that make a real difference.

Please note that this is a county-wide role and will involve travelling.
