Zusammenfassung

Whitdale Care Home, operated by West Lothian Council, is seeking a Social Care Practitioner. You will deliver high-quality personal care to 32 older residents and contribute to care planning, reviews and daily progress notes.

You will use the EMAR system for medication administration and work as part of a team to ensure person-centred care. SSSC registration is required within six months.

Qualifikationen

• SVQ III in Health and Social Care or equivalent qualification required.
• Registration with SSSC within six months of start date.
• PVG Scheme check required before offer.
• Experience with older people and dementia care is desirable.
• Ability to communicate effectively with residents, relatives and staff.

Aufgaben

• Provide direct personal care and support per individual care plans.
• Contribute to assessment, planning, delivery and reviews of care.
• Assist with medication administration using EMAR system.
• Collaborate with residents, families, carers and professionals.
• Participate in handovers, supervision and development reviews.
• Ensure care practice aligns with policies and promotes dignity and respect.
