The South Derbyshire South Adult Social Work Team, based in Swadlincote, is seeking a full-time Social Care Practitioner to join our friendly, community-based team of Social Care Practitioners and Social Workers.
Our team is highly supportive and operates on a largely agile, flexible basis across Swadlincote and surrounding villages of Derbyshire. We utilize link worker and champion roles to carry out Care Act (2014) face-to-face and telephone assessments and reviews.
The successful candidate will have direct experience working with disabled and older people in social care or health care settings. They should demonstrate a person-centred and strengths-based approach to achieve the best outcomes for individuals. Awareness of current legislation, policies, and procedures underpinning good practice is essential. The role includes assessing and providing equipment to support independence and safety at home.
The candidate will contribute to the team’s responsibility to work closely with statutory and voluntary organizations to plan safe and timely discharges from hospitals.
This role requires duty cover and the ability to respond to changing situations, effectively triage and assess incoming work to support adults and their carers, who may require urgent responses.
