Job summary

Wiltshire Council is seeking a Social Care Practitioner to deliver person-centred assessments for adults with learning disabilities, helping them achieve independence and wellbeing in the community. You'll promote independence, work within multi-professional teams, and manage caseloads in a fast-paced environment.

The role is based locally with hybrid office/field requirements and requires strong communication skills.

Salary:£31,537 - £33,699 pro rata

Hours per week:30 hours

Our Learning Disabilities and Autism Teams are locality based and offer social work and occupational therapy services to those who require longer term social care support. We work with individuals and communities to promote resilience, independence and wellbeing.

The key responsibilities of a Social Care Practitioner are to provide person centred and outcome focused assessments to identify the support required to enable customers to make positive changes in their lives. You will promote customer independence, choice, and well-being in the community, using universal services and support where appropriate.

This role is fast-paced so you should be able to manage caseloads effectively and have excellent communication skills to be able to work collaboratively with multi professional teams.
