Job summary

North Yorkshire Council is recruiting a full-time Social Care Coordinator to join the Adults Assessment Team covering the Selby area. You will support adults and carers to achieve person-centred outcomes, promoting independence and wellbeing in line with legislation and best practice.

The role requires confident English communication, collaboration with health partners, and flexible county-wide working. You will maintain records, contribute to safeguarding, and develop support plans with value

Qualifications

• Experience of working with adults with care and support needs.
• Strong communication skills with adults, carers, colleagues and partner agencies.
• Ability to manage challenging conversations professionally and sensitively.
• Good organisational and IT skills with accurate record-keeping.

Responsibilities

• Undertake timely assessments and reviews for adults with care and support needs and carers, in line with legislation, policy, and best practice.
• Promote strengths-based, asset-based and person-centred approaches to support independence and wellbeing.
• Provide information, advice and guidance regarding services, support options and preventative services.
• Support adults and carers to access direct payments, self-directed support and other funding opportunities.
• Work creatively with adults and carers to develop support plans that meet assessed needs whilst ensuring value for money.
• Arrange and review care and support packages and work closely with Brokerage and partner agencies.
• Assess for and arrange simple equipment and adaptations to support independent living.
• Work collaboratively with colleagues across Health and Adult Services, partner organisations, voluntary agencies and advocacy services.
• Maintain accurate, timely and professional electronic case records.
• Support safeguarding responsibilities, including acting as a Safeguarding Adults Alerter and contributing to safeguarding enquiries where required.

Job description

North Yorkshire Council is looking to recruit a full time enthusiastic and motivated Social Care Coordinator to join our Adults Assessment Team covering the Selby Area.

This is an exciting opportunity to work within a supportive and experienced team, supporting adults aged up to 75 with care and support. You will play a key role in promoting independence, wellbeing, and person-centred outcomes for older people and their carers.

The role involves spoken communication; therefore, a confident use of the English language is required.

There will also be an expectation of flexible working across the county to meet the needs of the service. Social Care Coordinators are based within the Assessment Team and may occasionally be required to support neighbouring areas or rotate between teams to support continuous professional development and service delivery.

As a Social Care Coordinator, you will:

We are seeking individuals who:

• Have experience of working with adults with care and support needs.
• Are passionate about promoting independence and positive outcomes.
• Can communicate effectively with adults, carers, colleagues and partner agencies.
• Are able to manage challenging conversations professionally and sensitively.
• Can work independently whilst also contributing positively within a team environment.
• Have good organisational and IT skills and are able to maintain accurate records.

Qualifications

Applicants must either:

• Hold a Health and Social Care NVQ Level 3 Diploma (Adults), or
• Be willing to work towards this qualification following appointment.

Equivalent qualifications accepted include:

• NVQ Level 3 in Promoting Independence
• CQF Level 3 Diploma in Health & Social Care
• NVQ Level 4 in Care

The following professional qualifications are also accepted:

• Occupational Therapy qualification

Professionally qualified applicants must hold registration with the relevant professional body.
