Are you passionate about supporting people to remain independent and achieve the best possible outcomes?
We are looking for a motivated and enthusiastic Social Care Assessor (SCA) to join our Early Intervention and Discharge Social Work Team.
Working across acute hospital and community settings, you will play an important role in supporting safe and timely discharge and helping people regain or maintain their independence. You will work closely with individuals, families, carers and colleagues across health and social care to develop proportionate, person-centred plans that build on people's strengths and existing support networks.
You will manage a varied caseload, undertaking Care Act assessments, Mental Capacity Assessments, Best Interests discussions and meetings, alongside participating in the team's duty rota. You will need to work confidently within agreed timescales and performance and budgetary frameworks while ensuring the individual's voice, wishes and outcomes remain central to your practice.
We are particularly interested in applicants who embrace strengths-based practice, are confident having outcome-focused and preventative conversations, and are willing to think creatively about how people can be supported within their families, communities and local networks.
You will be joining a busy but supportive service where your ideas and professional contribution will be valued. We encourage colleagues to question and improve how we work and to contribute to the ongoing development of our Early Intervention and Discharge service.
In return, we offer flexible working arrangements, regular professional supervision and structured learning and development opportunities to support your continued professional growth.
We are looking for someone who is passionate about making a positive difference to adults and their families and who is committed to promoting independence, choice and control.
You will have experience of working with adults who require health and social care support, alongside a good understanding of Care Act assessments, reviews and care and support planning. A sound understanding of the Care Act 2014 national eligibility criteria and how these are applied in practice is essential.
You will take a strengths-based, person-centred and outcome-focused approach, recognising what people can do and working creatively with individuals, families, carers and community networks to achieve sustainable outcomes and maximise independence.
As this is a busy and fast-paced service, you will need to be organised, adaptable and able to prioritise effectively. You will be comfortable making decisions within your role, working independently when required and collaborating effectively with colleagues across health, social care and the wider multidisciplinary team.
Strong communication skills and professional curiosity are important. We want someone who is willing to ask questions, positively challenge established ways of working and contribute ideas that improve outcomes for individuals and the wider service.
Your practice and behaviours will reflect our EPIC values, and you will demonstrate a commitment to continuous learning, professional development and improving the way we work.
You will receive regular professional supervision and ongoing support and development as part of a welcoming and committed team.
An enhanced DBS check with children/adults barred list check will be required
Flexibility will be required to work outside core hours as and when required
You will have flexibility in terms of hybrid working at home and in the office
