Cheshire East Council is recruiting a Social Care Assessor to undertake person-centred and strengths-based assessments in homes, hospitals or care homes, aiming to improve resident outcomes across Cheshire East. We are recruiting for 2 full-time posts (37 hours) and 1 part-time post (30 hours) in the East Short Term Services based in Macclesfield

Qualifications

• Experience of working with a variety of customer groups.
• Experience of working closely with Health professionals.
• Experience of supervising/mentoring trainee staff.
• Understanding of human development, crisis intervention.
• Up to date knowledge of relevant legislations e.g. NHS and Community Care Act.

Job description

Social Care Assessor

SCA should be able to undertake person centred and strengths-based assessments either within the individual's own home, hospital or Care Home, with the aim of improving client outcomes for the residents of Cheshire East. Additionally, this post will provide a wide range of learning and training opportunities and includes integrated working across the Health and Social Care sector. This post is an excellent opportunity to progress your career.
