Job summary

Forward Trust is seeking a Smoking Cessation Practitioner in East Kent to help embed stop-smoking support within substance misuse services and improve health outcomes for service users.

You will deliver Very Brief Advice, support access to Nicotine Replacement Therapy, and use Power BI dashboards to monitor progress while collaborating with staff and partner organisations to embed cessation into care planning and continuity of care.

Qualifications

• Experience in substance misuse and/or mental health services is desirable.
• Ability to work with diverse populations and partner organisations.

Responsibilities

• Using Power BI data dashboards to monitor smoking status and identify intervention opportunities.
• Delivering Very Brief Advice and smoking cessation interventions to service users.
• Supporting access to Nicotine Replacement Therapy and providing behavioural support.
• Delivering training to staff to increase confidence in cessation advice.
• Developing stop-smoking campaigns and awareness activities for target populations.
• Monitoring outcomes and completing monthly reporting per service requirements.
• Collaborating with local stop-smoking providers to embed referral pathways.
