Job summary

City Health Care Partnership CIC is seeking an experienced Smokefree Service Coordinator to collaborate with GP practices, Primary Care Networks and community partners to promote and deliver stop smoking services across local communities.

You will monitor outcomes, support social prescribing, and help build effective local partnerships with a focus on reducing health inequalities and improving long-term health.

Qualifications

• Degree-level education or equivalent knowledge, skills and experience.
• National Centre for Smoking Cessation and Training (NCST) qualification.
• Strong knowledge of stop smoking issues and related health inequalities.
• Experience coordinating or supervising staff and developing systems or processes.

Responsibilities

• Build effective relationships with GP practices, Primary Care Networks and community partners.
• Promote Smokefree services and encourage people to access support.
• Deliver stop smoking interventions and help establish community clinics.
• Monitor outcomes and gather feedback to identify opportunities to improve services.
