Job summary

Overview

Central Cheshire Integrated Care Partnership (CCICP) deliver NHS Community Services across Cheshire. We are a dynamic, future-facing organisation passionate about delivering the highest quality healthcare while collaborating with community partners. We are seeking an enthusiastic Band 6 Community Occupational Therapist to join the SMASH Care Community/CCICP team on a permanent basis (with a fixed-term option for 18 hours per week).

You will be part of a motivated multidisciplinary team, with access to in-service training and opportunities to participate in continuous improvement. The main office base is Alsager Medical Centre, with the option to hotdesk across the SMASH footprint.

To be considered you must be HCPC registered and have completed Band 5 competencies. Clinical educator and preceptorship training is desirable. We also support apprentices within the team. A current driver’s licence and access to your own vehicle are required. There is an opportunity to work flexibly with remote working and access to a laptop and mobile phone.

Responsibilities
  • Clinical
    • Manage a defined complex patient caseload autonomously within a multidisciplinary team.
    • Contribute to service delivery in inpatient wards, outpatient and community clinics, and patients’ homes.
    • Respect diversity and provide culturally sensitive care; use advanced communication skills with patients and carers.
    • Gain consent for treatment, address barriers to adherence, and comply with the Mental Capacity Act.
    • Undertake specialist clinical assessment and develop evidence-based, individualised treatment plans in agreement with the patient.
    • Deliver individual or group interventions, monitor progress, and adjust plans to achieve goals.
    • Provide written and oral information to patients and carers; maintain accurate documentation and timely reporting to referrers.
    • Collaborate with other health and social care professionals to ensure integrated care and high-quality outcomes.
    • Attend MDT meetings, ward rounds, and keep up-to-date with professional standards.
    • Ensure safe use of equipment and provide education to staff, patients and carers as required.
    • Identify equipment and adaptation needs; manage related paperwork and stock control as required.
    • Participate in on-call and/or weekend rosters as service needs require; prescribing should align with the medicines management committee guidance.
  • Governance
    • Maintain awareness of current professional standards, policies and guidelines; ensure compliance.
    • Support implementation of national and professional guidelines (e.g., NICE).
    • Be a member of a relevant professional association and attend regional/national meetings as required.
    • Ensure compliance with Trust policies and report concerns to senior management as needed.
  • Managerial/Leadership
    • Manage a specialist caseload with effective time management and organisation.
    • Coordinate junior staff and assistants to meet service priorities; provide clinical leadership and supervision.
    • Conduct appraisals and contribute to recruitment, induction and training for junior staff and assistants.
    • Contribute to service improvements, objectives and project delivery; represent the department in relevant groups.
    • Maintain a healthy, safe, and compliant working environment with accurate documentation and reporting of concerns.
  • Education/Learning
    • Plan, deliver and evaluate training for health care professionals, patients and carers.
    • Supervise junior staff and assistants, implementing induction and competency programmes; support student placements.
    • Promote health education across organisational boundaries and participate in in-service training and peer review.
    • Commit to ongoing professional development, including appraisal and maintaining a portfolio of learning.
