Job summary

University Hospital Southampton NHS Foundation Trust is seeking a dedicated Support Worker to assist in the Skin Cancer CNS-led pathway. You will support patients with access to personalised care, coordinate care and investigations, and help with triage of patient phone calls within the Skin MDT.

Job description

The successful candidate will be responsible for independently but under the guidance and supervision of a Clinical Nurse Specialists (CNS), to enable and support patients who have a diagnosis of a skin cancer. The post holder will also be part of the wider Skin multidisciplinary team (MDT).

The support worker will act as a point of contact for patients with skin cancers. They will be able to support patients with access to personalised care, including holistic needs assessment (HNA), care planning and health and wellbeing information.

The role will involve supporting the CNS teams with triage of their patient phone calls, coordinate care and investigations.

Another key aspect of this role will be to manage and support the delivery of personalised care which will be provided for patients as they complete treatment and transition into follow up care. The CSW with the CNS will scope the need and feasibility of other supportive self-management interventions - for example, 'Health and Wellbeing events'.
