Shift Pattern: Normal shift hours are 08:00-20:30. Shifts will include Bank Holidays and weekend working.
Croydon Health Services is part of a new Alliance Partnership with local health and social care providers. The LIFE service is an integrated model aimed at supporting individuals to remain independent and healthy, prevent hospital admissions, and shorten hospital stays.
The Single Point of Assessment (SPA) is a hub that receives all community referrals 7 days a week. The SPA clinician primarily coordinates and manages these referrals, communicating with patients via telephone and supporting rapid response assessments by appropriate clinicians.
This role is office-based with limited face-to-face patient contact. The position may close early if sufficient applications are received. If you do not hear within four weeks of the closing date, consider your application unsuccessful.
