Job description

Position: District Nurse / Nurse Practitioner - Single Point of Assessment

Note: Previous applicants need not apply.

Shift Pattern: Normal shift hours are 08:00-20:30. Shifts will include Bank Holidays and weekend working.

Living Independently for Everyone - LIFE

Croydon Health Services is part of a new Alliance Partnership with local health and social care providers. The LIFE service is an integrated model aimed at supporting individuals to remain independent and healthy, prevent hospital admissions, and shorten hospital stays.

The Single Point of Assessment (SPA) is a hub that receives all community referrals 7 days a week. The SPA clinician primarily coordinates and manages these referrals, communicating with patients via telephone and supporting rapid response assessments by appropriate clinicians.

Role Responsibilities:
  1. Coordinate and manage community referrals via SPA.
  2. Triaging calls for the Rapid Response Service and supporting assessments by nurses, occupational therapists, or physiotherapists.
  3. Clinically triage all community health referrals and LIFE service referrals.
Candidate Requirements:
  • Experienced District Nurse or Nurse Practitioner with extensive community service knowledge.
  • Clinical triaging expertise, excellent communication skills, and strong time management and prioritization abilities.
