Single Point of Assessment and Rapid Response Triage Nurse (SPA)

Join to apply for the Single Point of Assessment and Rapid Response Triage Nurse (SPA) role at Croydon Health Services NHS Trust.

Normal shift pattern is 08:00-20:30.

Shifts will include Bank Holidays and weekends.

Croydon Health Services are part of a new Alliance Partnership with other health and social care providers in Croydon. The LIFE service is an integrated model supporting people to remain independent and healthy, aiming to prevent hospital admissions and shorten stays.

The SPA role involves managing community referrals received 7 days a week, triaging calls for the Rapid Response Service, and supporting assessments by appropriate clinicians. The role is office-based with communication primarily via telephone.

We highly recommend visiting the service before applying. The interview date is scheduled for 6th June 2025.

Responsibilities include coordinating community service referrals, triaging calls, and supporting timely assessments by nurses, occupational therapists, or physiotherapists.
