Shifts will include Bank Holidays and weekends.

Croydon Health Services are part of a new Alliance Partnership with other health and social care providers in Croydon. The LIFE service is an integrated model supporting people to remain independent and healthy, aiming to prevent hospital admissions and shorten stays.

The SPA role involves managing community referrals received 7 days a week, triaging calls for the Rapid Response Service, and supporting assessments by appropriate clinicians. The role is office-based with communication primarily via telephone.
