This is an exciting opportunity for you to work within our Continuing Health Care team. You will be working with Health colleagues to support people who are being assessed for Continuing Health Care eligibility and Joint-Funded Support, to improve outcomes in a person-centred way.

We are looking for dedicated Service Advisors with strong interpersonal and communication skills, as your role will include liaising with other professionals and you will also form part of a duty system responding to enquiries from colleagues, ICB partners, and on occasion members of the public. You will also need good attention to detail and analytical skills as you will be updating our data systems and records. This will require you to have good skills in managing your workload demands and meeting deadlines.

You will have excellent IT skills, including good knowledge of applications and systems including Excel and Sharepoint, and ideally experience of case management systems such as Mosaic. You will also have knowledge of Continuing Health Care, community care services and relevant legislation, including the National Framework, Section 22 of the Care Act 2014, the Mental Capacity Act (2005) and Deprivation of Liberty Safeguards.

Ideally you will have at least two years’ experience of work within Social Care or Health, though this is not essential. You will have experience of working with the public and partners, keeping records and constructing reports.

The role will enable you to work flexibly through the hybrid working model, with the working week split between home and the office. A comprehensive induction programme and all relevant training will be provided.

Interviews will be scheduled for week commencing 28th September.

This post is also available as a secondment opportunity to existing Nottinghamshire County Council employees. You must seek approval from your line manager before applying.

Appointment to this post is subject to a Disclosure and Barring Service check.
