Wirral Community Health and Care NHS Foundation Trust invites applications for a Nurse Clinician in the Teletriage team. The role focuses on acute clinical assessment within care homes and aligns with the National Urgent Care Framework.

You will provide senior clinical support to urgent community response staff and coordinate with acute trusts to avoid unnecessary admissions. The post holder will work autonomously, with established pathways to assess patients and decide if further face-to-face

Qualifications

• NMC registration (or HCPC if applicable).
• 3+ years post-registration experience.
• Knowledge of acute, chronic and terminal illness, wound management, and palliative care.

Responsabilités

• Provide autonomous clinical assessment for acutely unwell patients.
• Support care home staff in determining optimal clinical pathways.
• Facilitate early discharges and prevent unnecessary hospital admissions.
• Collaborate with multi-disciplinary team to deliver senior clinical support.
