Job summary

Wirral Community Health and Care NHS Foundation Trust is seeking a Nurse Clinician to join the Urgent Community Response team. You will assess acutely unwell patients in care home settings and provide senior clinical support to urgent community staff to prevent unnecessary hospital admissions.

The role requires autonomous practice, experience in rapid response, and collaboration with acute trusts. Band 7 salary applies, with NHS terms and conditions and full-time hours.

Qualifications

First Level Nurse with current registration with NMC or Experienced Community Paramedic with HCPC registration.
Masters or equivalent experience.
Post Graduate qualification in rapid response/community nursing/emergency department/acute medical or equivalent experience.
Evidence of recent Professional Development.

Responsibilities

Provide a wide range of acute, nursing and social care assessments.
Support care home staff in determining best clinical pathways.
Autonomous practitioner with capacity to discharge at first point of contact when appropriate.
Make clinical judgments and decisions requiring prompt action.
Work in a multi-disciplinary team to share knowledge and skills.
