The service supports independence and community involvement and requires a proactive, resilient candidate capable of working both independently and with a team.

Qualifications

• Willingness to work towards Level 3 Diploma in Health & Social Care.
• Ability to work unsociable hours on a rota basis.
• Drivers desirable but not essential.
• Committed to maintaining high standards and practice.

Responsibilities

• Lead and supervise a team of Support Workers.
• Deliver and oversee person-centred support with updated care plans.
• Coordinate activities, appointments and community engagement.
• Maintain thorough records, incident reports and reviews.
• Participate in Senior On-Call rota to respond to needs as required.
