Job summary

Qualifications

• Recognized social work qualification (BA/MA).
• Current registration with Social Work England.
• Experience with young people with disabilities.
• Knowledge of legislation related to preparing for adulthood.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive assessments to identify needs and aspirations of young people with disabilities.
• Develop and implement personalized support plans to facilitate transition to adulthood.
• Collaborate with multi-disciplinary teams to coordinate services and resources.
• Guide families and caregivers to support clients effectively.
• Monitor progress and adjust plans to changing needs.
