Job summary

Liquid Personnel is seeking a Senior Social Work Practitioner for the Preparing for Adulthood Team in Shropshire. This role requires experience in supporting young adults during their transition to independence, emphasizing person-centered care.

Key responsibilities include assessing needs, developing support plans, and coordinating multi-agency services. Candidates must hold a relevant social work qualification and be registered with Social Work England.

Qualifications

• Experience within a statutory setting working with young people transitioning to adulthood and/or adults, including safeguarding.
• Strong understanding of relevant legislation relating to adult social care, safeguarding, and transition services.

Responsibilities

• Work in partnership with young people, adults, and their families/carers to assess needs and develop support plans.
• Undertake statutory assessments, safeguarding activity, and care planning.
• Coordinate multi-agency services, working closely with health, education, housing, and partner organisations.

Skills

Strong assessment, analytical, and care planning ability

Ability to manage complex caseloads and competing priorities

Excellent communication and multi-agency working skills

Education

Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW

Must be registered with Social Work England

Experience using electronic case management systems
