Hertfordshire County Council is seeking a Senior Social Worker (Adults) for the Transforming Care Team, 0-25 Together Services. You will develop discharge pathways from psychiatric settings, work with a multi-disciplinary group, and support adults with complex needs to live safely in the community.

Role requires leadership in risk assessment, advocacy collaboration, and working across the county with flexible arrangements and travel.

Qualifications

• Qualified and registered Social Worker with professional standards.
• Experience in learning disabilities, autism, mental health and relevant legislation.
• Experience in assessing capacity and best interests, DoLS knowledge.
• Experience collaborating across MDTs, families, advocates and health/social care leads.
• Excellent communication and organisational skills.
• Able to work autonomously, Smart Working, and travel within/out of county.
• Strong skills in assessment and report writing.
• Experience in complex case management and high-risk situations.
• Driving licence and access to a vehicle for county travel.

Responsabilités

• Develop discharge pathways from psychiatric hospitals and contribute to preventing hospital admissions.
• Provide personalised support to adults with complex care needs in a multi-disciplinary setting.
• Work within an All Age approach across learning disability and autism and liaise with advocacy, health and commissioners.
• Support safeguarding, risk management and ensuring people stay at the centre of care.
