Transforming Care is a national programme implemented by NHS England to transform the care and support for children, young people and adults with a learning disability and/or autism who may be in hospital/at risk of hospital admission, or possibly in contact with the Criminal Justice System.
The Hertfordshire Transforming Care Team are proud to work to enable people to live ordinary lives in the community despite significant barriers and challenges. Do you have what it takes to keep people at the centre of your approach as you work within a supportive and experienced team to prevent admission, support discharge and build community capacity as suitable alternates to hospital?
You will work at the forefront of best practice to deliver personalised support to vulnerable adults with the most complex care needs. You will have the skills necessary to develop discharge pathways from psychiatric hospitals and in prevention work to avoid inappropriate admission to hospital. Your work will be adult focused but within a team that offers an All Age approach across the fields of learning disability and autism. You will work alongside wider professional agencies including advocacy, health and local and national commissioner groups to effect change.
Essential to the role is the ability to see the potential in people and to take a creative and proactive approach to risk and problem solving despite being challenged to the contrary. You will understand the complex and often conflicting matters of enabling ordinary lives whilst protecting people including the public from harm, and the role legal frameworks play. You will be able to progress and keep people at the centre in ‘stuck’ cultures and systems.
In return, we will offer excellent learning and varied development opportunities. A protected caseload to deliver intensive and personalised care, regular supervision (including, if relevant, access to clinical specialism supervision), and an approachable and supportive team who are keen to keep their ideas fresh and learn together.
