Qualifications

• Support young people through their transition from Children to Adult services.
• Utilise EHCPs, SEND frameworks and safeguarding procedures.
• Undertake strengths-based assessments and develop person-centred support plans.
• Work with partner agencies to deliver coordinated support.
• Apply Children’s and Adults’ legislation confidently and effectively.
• Support young people with complex needs to achieve independence.
• Contribute to the ongoing development of the Preparing for Adulthood service.
• Undertake safeguarding responsibilities and support risk management.
• Apply the Mental Capacity Act and Continuing Healthcare frameworks where appropriate.
• Maintain accurate case records and professional documentation.
