Sanctuary Personnel is seeking an Adults Social Worker for the Preparing for Adulthood Team in Warrington. The role supports young people as they transition into adulthood under an ongoing locum arrangement with full-time or part-time flexibility.

Qualifications

• Demonstrate evidence of ongoing Continuing Professional Development.

Responsibilities

• Undertake strengths-based assessments, reviews, and support planning in accordance with the Care Act 2014.
• Assist young individuals with Learning Disabilities and Autism through their transition from children's to adult services.
• Develop, implement, and review person-centred care and support plans that encourage independence and choice.
• Complete Mental Capacity Assessments and contribute to decision-making under the Mental Capacity Act 2005, including Court of Protection processes when necessary.
• Identify, assess, and manage risks while ensuring safeguarding concerns are addressed following local and national procedures.
