Job summary

Service Care is seeking an experienced CSW - Senior Social Worker for Children Looked After & Leaving Care (GRADE K-L) on a two-week contract in Milton Keynes. You will manage a caseload, conduct holistic assessments and pathway planning, and work with young people, foster carers and agencies to secure supportive outcomes.

The role focuses on statutory leaving care support, education and independence planning, health and housing coordination, and safeguarding within a multi-agency framework.

Qualifications

• Professional social work experience with safeguarding
• Experience with children looked after, care leavers or young people transitioning to adulthood
• Knowledge of pathway assessments and pathway plans
• Excellent communication, recording and multi-agency partnership skills
• Ability to advocate for young people’s voice and choices

Responsibilities

• Complete and review Pathway Assessments and Pathway Plans with young people and relevant professionals.
• Undertake visits to children looked after and care leavers, ensuring they are settled, supported and able to focus on their goals.
• Support health, dental care, immunisations, emotional wellbeing and access to appropriate services.
• Arrange and participate in Personal Education Plan meetings, supporting education, training and employment outcomes.
• Provide advice and guidance regarding accommodation, independent living, financial assistance and tenancy management.
• Help young people develop practical life skills and prepare for independent adult life.
• Promote participation, family and social relationships, community involvement, volunteering and local opportunities.
• Maintain accurate case records and comply with statutory duties, council policies, procedures and financial regulations.
• Liaise with health, housing, education, youth justice and other agencies to coordinate effective support.
