Job summary:

We are currently seeking a dedicated and experienced CHC (Continuing Healthcare) Senior Social Worker to join our team in Cambridge. As a CHC Senior Social Worker, you will play a crucial role in assessing, planning, and coordinating healthcare services for individuals with complex needs, ensuring they receive the appropriate support to live fulfilling lives within the community.

Key Responsibilities:

  • Conduct comprehensive assessments to determine eligibility for Continuing Healthcare funding.
  • Work collaboratively with multidisciplinary teams to develop and implement care plans that meet the needs of individuals and their families.
  • Advocate for clients and ensure their voices are heard throughout the care planning process.
  • Provide support and guidance to individuals and families navigating the healthcare system, including signposting to relevant services and resources.
  • Monitor the effectiveness of care plans and make adjustments as necessary to ensure optimal outcomes.
  • Maintain accurate and up-to-date documentation in accordance with regulatory standards and organizational policies.

Requirements:

  • Bachelor's degree in social work or related field.
