Exciting locum opportunity for a Senior Social Worker – Joint Emergency Team in Woolwich, earning £36 per hour on an hourly basis! You will be working with The Joint Emergency Team (JET), a renowned multi-professional team consisting of nurses, occupational therapists, physiotherapists, general practitioners, and expert social workers. JET is committed to providing essential medical and social care interventions to support individuals in their homes across the Royal Borough of Greenwich.

What you will do:

• Utilise your expertise in Strength Based Approaches to co-produce care solutions with a person-centred focus.
• Work flexibly, including at least one late shift per week from 12:30 PM to 8:30 PM and one weekend in four—don’t worry, you’ll get Thursday and Friday off for those weeks.
• Assist with referrals received through various entry points including GPs, 111, and the Virtual Ward.
• Complete Continuing Health Care assessments for borough residents without an allocated social care member of staff, with support and training provided.
• Engage in a fast-paced, integrated team setting where no two days are the same.
• Help maintain JET’s goal of supporting individuals at home to prevent unnecessary hospital admissions.
