Job summary

West Hertfordshire Teaching Hospitals NHS Trust is seeking an experienced Rotational Advanced Pharmacist to join our Pharmacy team on a fixed-term basis. This senior clinical role will span Medicine, Care of Older People and the Acute Admissions Unit, providing expert pharmaceutical advice and supporting safe med usage.

You will work with multidisciplinary teams, contribute to service development and medicines optimisation, and support the development of junior pharmacy colleagues.

Qualifications

Post-qualification hospital pharmacy experience.
Experience in Older People, Medicine and Acute Admissions.
Experience in multidisciplinary teams.
Experience training Pre-reg Pharmacists and junior staff.
Audit/research and development experience.

Responsibilities

Deliver and evaluate high-quality clinical pharmacy service for Older People.
Provide expert pharmaceutical advice and ensure medicines safety.
Participate in clinical audit and practice research.
Train pharmacy staff and multidisciplinary team.
Deprescribe as independent prescriber and train on deprescribing tools.
